Passport Photo

Passport Application Fee $225 for adults. $195 for minors under 16 years of age 

Proof of international travel (If you don't have it, we can assist you with the required documents)

Provide one of the following  evidence of U.S. Citizenship 

• Fully-valid, undamaged U.S. passport (may be expired)

• U.S. Birth Certificate (original copy)

• Certificate of Naturalization 

Bring a photocopy of U.S. Citizenship (front and back)

Current Photo ID

Bring photocopy of ID (front and back)

Fill Out Form

• DS82 Form

Passport Photo

Passport Application Fee $190 

Proof of international travel (If you don't have it, we can assist you with the required documents)

Submit your most recent U.S. passport

Include name change documents (if applicable)
